No hard copy bids or facsimile bids will be accepted. Bid Package SUB#04 generally consists of providing labor, equipment, and materials related to the scope of work for the placement of asphalt concrete paving per the limits indicated on the project drawings:

1. 1. Work consists of performing Full width 5鈥� paving on Airway Road and Walker Road.
   2. Work consists of Installing Permanent trench patching on Stoltz Hill Rd, Vaughn
      Lane, 5th Street, Crowfoot Road and South Main Street.
   3. Work consists of install 2鈥� overlay on Stoltz Hill Rd, Vaughn Lane and 5th Street.
   4. Work Consists of installing 2鈥� AC inlay on Stoltz Hill RD, Vaughn Lane and
      Crowfoot Rd
   5. Work consist of installing 3鈥� Commercial AC Driveways on Airway Rd and 3鈥�
      Private AC driveway sections from back of new concrete driveways to existing AC
      Driveway on Walker Rd.
   6. For Bidding Purposes Assume 7 Mobilizations (1 each for Airway Rd, Stoltz Hill Rd,
      Vaughn Lane, 5th street, Crowfoot Rd, Walker Rd, South Main Street.

Locations and materials are as indicated and specifi ed in the project plans and
specifications. The bidding documents include the following: Invitation to Bid,
Instructions to Bidders, Bid/Proposal Forms, City of Lebanon General聽 Conditions, Sample Emery & Sons Subcontract, Emery & Sons Health & Safety Manual, Plans, and Specifications.
